Posterior incisions are made bilaterally at the L4-5 level. A microscope is used during surgery. The facets at L4 and L5 are removed, pedicle screws are placed, the disc material is removed, interbody cages filled with bone graft are placed, foraminotomy is performed and rods are applied to complete the hardware.
